A checklist added to make sure all signatures are done within the first 30-day period. For the release of information, I suggest doing a spreadsheet to keep track of all the request for release of information so this can be work on an sent in a timely manner. In the matter of the incident reports, we need to see where the error is occurring. For core measure audit the recommendation I have ensure all patients that has abnormal chest findings should have ECG immediately this can save someone’s life if it is caught early. The areas that I see need to be improved is Release of information and DC Records Completion for signature. Two areas need much work on the Release of Information and should be signed within a 30-day period on site and 60-day off site, that should be more than enough time to have the information release. DC record completion for signature, I just can’t comprehend why would there not be a signature sign within the 30-day period. 2
