Description of Issue or Problem Proper management of fluids is crucial for dialysis patients. The buildup of excess fluid between dialysis treatments can lead to complications such as strain on the heart and lungs. In severe cases, it can even result in hospitalization or death (Loutradis et al., 2021). The dialysis clinic where this problem was identified regularly tracks patient outcomes related to fluid overload, including post-dialysis weight, blood pressure, and hospitalizations. Data from this clinic has revealed that many patients find it challenging to follow fluid restrictions, particularly those with other health conditions like heart failure, which leads to frequent hospitalizations and poor outcomes. A study has shown that closely managing fluid levels in dialysis patients has a significant impact on their chance of survival in the short term (Kim et al., 2022). It is crucial to educate patients about the importance of controlling their fluid intake and to ensure that they follow the guidelines. However, many patients struggle due to emotional and social barriers, making it hard emphasizes the role of nurses in assessing patients' ability to perform self-care and providing education and interventions to fill gaps in knowledge or capability. Orem's theory is well-suited to this issue because it focuses on empowering patients to take control of their health. Fluid management requires consistent, long-term behavioral changes, and Orem's framework offers strategies for supporting patients in developing self-care practices. By identifying patient knowledge or motivation deficits, nurses can implement targeted interventions such as education programs and personalized care plans. Research has shown that targeted education about fluid and dietary restrictions can be effective for patients. For example, a study by Sukartini et al. in 2022 found that patients who received this type of education were better able to follow their fluid management plans and experienced fewer complications. Nurses can use Orem's theory to customize their interventions to each patient's needs, making educational efforts more relevant and effective. In addition, continuous monitoring and support are crucial for the success of these interventions. Nurses should regularly assess patients' adherence to fluid restrictions and adjust the educational program as necessary.
