Mental Status Examination Casual attire, well-groomed, no motor abnormalities. Cooperative, good eye contact. Alert and oriented x 3. Speech is clear, normal rate and volume. Mood is depressed, irritable, and angry. Affect is appropriate, congruent to mood. Does not appear to respond to external stimuli. Memory grossly intact, fair concentration and attention. Thought processes are coherent with average intellectual functioning. Thought processes appear organized and content is appropriate to subject. Denies suicidality or homicidality. Select an appropriate screening tool for Kristin and provide a score for the tool. Screening tool: Bipolar spectrum diagnostic scale Score: 13 (moderate probability) b. Provide a rationale for the screening tool you selected. I selected Bipolar spectrum diagnostic scale (BSDS) to screen Kristin for bipolar disorder because this tool is useful in measuring the probability of the bipolar diagnosis. This tool alone is not sufficient for the diagnosis, but it tells you how likely patient may be diagnosed with bipolar disorder. I chose BSDS vs Mood Disorder Questionnaire (MDQ) because MDQ includes questions only regarding manic episodes. c. Determine the most likely diagnosis for Kristin based on the available information. Bipolar II disorder, current episode depressed When Kristin is not sad, she crafts for straight 3-4 days and nights. The energy level is higher than normal but not as high as manic episode and the episode last for 3-4 days including symptoms like decreased need for sleep, buying sprees, difficulty focusing for long periods and racing thoughts which meets the criteria for hypomanic episode. She currently has depressed mood, easily irritable, weight loss of 10lb, sleep disturbance, diminished ability to concentrate and feeling of worthless which has lasted for a month (> 2 weeks) meets the criteria for depressive episode. d.
