1. Identification of the Selected Measure a. Identify the selected performance measure. a. The performance measure that I would like to focus on is weight assessment and counseling for nutrition and physical activity for children/adolescents which is under the effectiveness of care domain. b. Analyze the selected performance measure for clinical significance and evidence base. a. Over the last few decades, the rate of childhood obesity has increased exponentially. Not only does obesity in childhood have instant effects on the body, but it also has long- term effects. A study by Brown et al. (2015), it was shown that intervening at the level of pediatrician, school, and family has success in preventing childhood obesity. c. Describe practical implications for practice. a. Intervening as a primary care nurse practitioner is the first step to addressing childhood obesity. Obesity in children, when left untreated, can lead to serious conditions later in life including cardiovascular disease, kidney disease, OSA, and hypertension to name a few (Smith et al., 2020). Due to these long-term effects caused by childhood obesity, it prompts advanced practice nurses to educate their patients on healthier food choices, create a plan for exercise, and follow up appointments to monitor progress. In my future practice, I aim to focus on non-medication-based remedies before going down the prescription route, especially for children. 2. Develop a quality improvement plan for implementing the selected performance measure in your current or future practice, including: i. Strategies for client education: a. Educating our patients and their family about the importance of weight/nutrition management by attending nutrition counseling sessions, meal planning strategies, and showing examples of healthy food choices. One of the biggest barriers to healthy living in recent years is physical activity and screen time. Educating families and the patients on increasing physical activity, no matter how small, and limiting screen time can make a difference even if it does not seem like it would. Supporting the patient and their family with these changes is important as well because without the support of others, the patient and family will, unfortunately, not continue with the changes (Rankin et al., 2015). 0320 RB/KK 4. The interventions that are implemented can improve client outcomes by helping maintain a healthy weight, nutritious diet, and active lifestyle. Improving the previously mentioned factors will prevent future health concerns including heart disease, type 2 diabetes, kidney disease and many more (Smith et al., 2020). b. Discuss how the interventions can improve cost savings for the practice. a. The interventions can improve costs savings for primary care practice because managing weight and forming proper nutrition plans decreases the risk of future health diagnoses. Decreasing the risk of future risk diagnoses lowers costs because chronic conditions are costly to treat since they are lifelong treatments (Thorpe & Joski, 2024). c. Discuss how the interventions can improve client ratings (an NP’s patient scorecard).
