The nurse and physician are frustrated and once again attempt to educate the family on the need for an emergent blood transfusion to keep Wanda alive. The family once again adamantly refuses. The nurse advises the family that there is a good chance Wanda will die in the next 24 hours if the transfusion is not given. The family still refuses. Four hours later Wanda is unresponsive and goes into cardiac arrest. Despite best efforts to revive Wanda, Wanda dies. The nurse and provider are extremely frustrated with Wanda and her family as they know this death could have been prevented. The above situation addresses the principle of respecting the patient’s autonomy. Healthcare professionals may believe that a particular treatment or intervention is in the best interest of the patient, even if the patient disagrees like in the case with Wanda. Balancing the principles of patient autonomy and beneficence can be ethically challenging, requiring careful consideration of the potential benefits and harms of different courses of action and causing moral distress for nurses and providers. Key stakeholders in this ethical dilemma are the patient, the patient’s family, the nurse caring for the patient, the provider caring for the patient, the nurse’s aides or patient care techs, the patient advocate for the hospital and the hospitals ethics committee. The key stakeholders in this situation will be impacted in different ways. The patient is impacted the most, as they are putting their own life at risk by following their religious beliefs and ultimately dying. Even though they are following their strong religious beliefs, I would concur that there is still personal distress the patient is confronted with. The nurse and provider, regardless of their beliefs in this situation, still need to support the patient with their decision and help the patient work through the distress. That is a fine line to walk as a healthcare provider – balancing the patient right to autonomy and the ethical principle of beneficence. The provider and nurse may struggle because the patients' values may conflict with their own cultural or religious beliefs, which can make it difficult for the provider and nurse to navigate the situation while still respecting the patient's choices. This situation can cause moral distress for the nurse and provider as they watch a patient die knowing they can prevent the death from occurring. The patient’s family is impacted by the death of the patient, even if it was the religious choice of the patient and the family supported that choice. It is still a loss of a family member and there is still grief to process. If the family did not share the religious beliefs of the patient or support the
