Doctor of Nursing Practice (DNP) prepared nurses are equipped to facilitate continuous improvement by conducting organizational assessments to identify gaps or issues affecting patient outcomes and integrating stakeholder management to promote safe and high-reliability performance (Hassmiller & Wakefield, 2022). This discussion aims to assess the process for evaluating opportunities for improvement in my selected practicum organization. Examining the Process for Assessing Opportunities for Improvement I plan to conduct my DNP project at the Tampa General Medical Group organization. The process I would follow to assess the organization of opportunities for improvement involves two significant steps. First, I would conduct a needs assessment to identify gaps in current practices and patient outcomes, which is crucial to developing targeted interventions (Ravaghi et al., 2023). For instance, I would use frameworks like Strengths, Weaknesses, Opportunities, and Threats (SWOT) analyses to identify external and internal factors affecting their patients' population's performance and determine specific areas for improvement. The practice problem has already identified increased incidences of uncontrolled hypertension among patients already diagnosed with the disease and under medication. Second, having identified the practice gap, I would initiate stakeholder identification, analysis, and management (Williams & Finkelstein, 2023). The approach would involve identifying relevant internal and external stakeholders involved in the quality improvement process, such as organizational administrators, health care providers, patients, and project mentors or preceptors. I would evaluate their roles and develop approaches to proactively engage and manage them in developing an action plan, acquiring resources, increasing buy-in, and aligning the improvement initiative with the organization’s needs (Williams & Finkelstein, 2023). Determining the Ultimate Goals for My DNP Project The proposed quality improvement initiative selected to address the high incidences of uncontrolled hypertension among patients in Tampa General Medical Group is the implementation of a Dietary Approaches to Stop Hypertension (DASH) diet program. The aim is to reduce patients’ systolic blood pressure (SBP) to < 140 mmHg and/or diastolic blood pressure (DBP) to ≤ 90 mmHg by promoting adherence to healthy diets, such as low-sodium foods, unsaturated fats, and vegetables and fruits, and minimizing risk factors for uncontrolled hypertension (Guo et al., 2021). The ultimate goals for my project are to increase patients’ adherence to healthy dietary practices, minimize incidences of uncontrolled hypertension, and improve overall health care outcomes by reducing disease-related complications and events like hospital readmissions.
