Methadone non-adherence in opioid treatment programs is a significant issue (Ertem & Duman, 2019), and it greatly affects treatment outcomes and quality of life for patients. Motivational interviewing is an evidence-based counseling approach that can be utilized to address this problem. It has been shown to be effective in fostering treatment adherence (Ertem & Duman, 2019). This strategy enhances adherence for several reasons, primarily by promoting patient engagement, motivation, and self-efficacy in managing various health conditions. The following paper will synthesize current research on the application of motivational interviewing to improve adherence to methadone treatment in individuals with opioid use disorder. Significance of the Practice Problem at the National Level Prevalence Opioid use disorder (OUD) is a significant health issue in the United States, affecting millions of Americans and leading to various health complications while also placing a considerable strain on the healthcare system. In 2022, an estimated 3.7% of U.S. adults (9,367,000) needed OUD treatment. Among these, 55.2% (5,167,000) received OUD treatment, and 25.1% (2,353,000) received medications for OUD. Most adults who needed OUD treatment either did not perceive that they needed it (42.7%) or received treatment that did not include medications for OUD (30.0%) (Dowell et al., 2024) Mortality rates The mortality rate associated with opioid use disorder (OUD) is alarmingly high, with most deaths resulting from overdoses. According to provisional data from the Centers for Disease Control and Prevention's National Center for Health Statistics, an estimated 107,543 drug overdose deaths occurred in the United States in 2023. This represents a 3% decrease from the 111,029 deaths estimated in 2022 (CDC, 2024). Notably, this is the first annual decline in drug overdose deaths since 2018. The issue of drug overdose continues to be concerning despite a reduction in the national rate, highlighting a persistent health crisis. Economic Impact The economic impact attributable to opioid use disorder is staggering, encompassing a wide array of expenditures related to healthcare costs, lost productivity and workforce implications, and criminal justice expenditures. According to recent estimates by the CDC, the total economic burden of opioid misuse in the United States exceeds $600 billion annually, with a substantial portion of this figure stemming from healthcare-related expenses. This figure reflects direct costs and highlights the extensive resources required to manage the consequences of OUD on the healthcare system. The value of life lost due to overdose deaths was $480.7 billion. Opioid use disorder accounted for $471.0 billion. Criminal justice spending accounted for $14.8 billion. Evidence Synthesis The following synthesis will review five recent studies that use motivational interviewing (MI) in different health contexts to examine its efficacy as an effective intervention for fostering methadone adherence.
