Document Overview

2 Project Model Unplanned hospital readmissions impose substantial financial burdens on both providers and patients. For providers, these readmissions can result in significant costs stemming from additional treatments, prolonged hospital stays, and potential penalties from healthcare payers. Patients also face increased medical expenses, disruptions to their recovery process, and a heightened risk of complications such as infections and medical errors due to readmissions. The risk for readmissions increases when the patient is discharged from the intensive care unit (ICU) without adequate monitoring and follow up care (Jiang & Hensche, 2023). Effective discharge planning based on a standardized approach and follow-up care are essential for improving ICU patient outcomes and reducing overall healthcare costs. Evidence-based practice (EBP) models provide a structured framework for addressing clinical problems such as unplanned readmissions by integrating the best available evidence, clinical expertise, and patient preferences. These models guide healthcare professionals in systematically searching for, appraising, and applying research findings to improve patient outcomes. This paper will provide an overview of the Johns Hopkins Nursing Evidence-Based Practice (JHNEBP) Model and discuss how it can be utilized to address the clinical question, β€œIn patients discharged from ICU units, does the implementation of a standardized discharge protocol compared to current discharge practices without a standardized protocol reduce readmissions over a 30 day period?” Description of the Model The Johns Hopkins Nursing Evidence-Based Practice (JHEBP) Model offers a structured approach to help nurses incorporate the most reliable evidence into their clinical practice. The model consists of three main phases. These phases are referred to as the PET process, which stands for the Practice Question, Evidence, and Translation. During the Practice Question phase, 4 consumption of already strained healthcare resources. The aim of this study is to determine if implementing evidence-based practice through a standardized discharge protocol can significantly reduce the risk of patient readmission by ensuring consistent and comprehensive post-ICU discharge care. Practice Application The JHNEP model can be used to systematically address the PICOT question and determine whether implementing a standardized discharge protocol improves patient outcomes in ICU settings. Applying the model to the practice question requires following the PET process (Dang et al., 2021). The initial phase of the plan is to develop a research question in the PICOT format. The population that the research question is focused on is patients discharged from ICU units. The intervention is the implementation of a standardized discharge protocol. The comparison will be the current discharge practice without a standardized protocol. The outcome measured will be a reduction in ICU readmissions, hospital readmissions, and improved patient satisfaction. The time for the research will be over 30 days. The second phase of the process is gathering, appraising, and synthesizing the evidence (Dang et al., 2021). A comprehensive literature review to identify studies, guidelines, and expert opinions on the impact of standardized discharge protocols in ICU settings is conducted followed by a critical evaluation of the quality and relevance of the identified evidence. This includes assessing study designs, sample sizes, and the validity
