TeamSTEPPS mentions communication improvement tools such and SBAR and “I PASS THE BATON” for hand-off (Agency for Healthcare Research and Quality [AHRQ], 2020). Barriers and facilitators are important to consider and address prior to implementation and can make or break a project (Wende et al., 2022). Some barriers I can foresee in your project include the ability to unify nurses to use the communication tool and training on the tool. Facilitators of your project may include “champions” who promote positivity and continual use of the tool and perform rounds with the nurses to ensure the communication tool is being done at hand-off.
