The DNP prepared nurse will also make use of a questionnaire or survey which will be administered among the key stakeholders to evaluate the effectiveness of the DSMES intervention in improving glucose control among the identified patient population. Use of the two approaches will help to regularly assess and reassess the progress achieved towards completion of the project and ensure all the goals are wholly met. The primary tool that will be utilized in monitoring the project activities will involve use of the Gantt chart. Use of the Gantt chart will help the DNP prepared nurse to plan all the project activities and develop a schedule for each of the project activities. Through use of the Gantt chart, it will be possible to evaluate the progress achieved in implementing the proposed intervention and ascertain whether the project activities are undertaken as per the formulated project schedule (Li et al., 2022). The move will make it easy to conduct week to week assessment to ensure all the project activities are carried out accordingly and thus prevent any delays that lead to delays in implementing the proposed intervention. The DNP prepared nurse will also rely on use of the formulated questionnaires or surveys to ascertain the impacts of the new intervention in addressing the identified practice gap and improving patient health outcomes. Ultimately, my formative evaluation plan will help me assess the effectiveness of the intervention and determine if it is meeting the goals set forth. By regularly monitoring progress and making necessary adjustments, I will be able to ensure that the intervention is successful in achieving its intended outcomes. I will undertake periodic reviews of the progress made by the intervention in order to check whether or not the desired outcomes are being achieved.
