Document Overview
the PPI dose as sudden withdrawal can cause rebound acid reflux. If symptoms persist, referral to gastroenterology is the appropriate next step (Fenstermacher & Hudson, 2024). Although PPIs are the recommended treatment option for GERD, this patient presented with no critical symptoms and denied aggravating factors. Implementing H2 blockers instead of PPI reduced the risk of altering stomach bacteria and placing this patient at risk for developing H. pylori infection but allows for immediate resolution of symptoms. It also allows the provider to determine if the heartburn was an unresolved intermittent complaint or a chronic issue that needs a further workup. I think understanding that not every complaint needs to be addressed with the most aggressive treatment option is critical for nurse practitioners because sometimes those options come with unforeseen risks. This patient may not require an 8 week regimen of PPIs to treat her heartburn symptoms, however if the 14 days course does not alleviate her symptoms, then PPI treatment is still an option without having to increase PPI dosage or changing to an alternative PPI medication.
