There are several gaps that emerge during care transition and these gaps could hurt continuity of high-quality care. One gap in patient transition inadequate patient education for older adults, particularly those without a family caregiver. This inadequate education is caused by memory decline or by ageism attitude mostly from the patients. According to Nemiroff (2022), ageism not only contributes to a lower quality of life for older patients, but can also result in missed or delayed diagnoses, more emergency room visits, more frequent hospitalizations and a shorter lifespan. Additionally, lack of a single point person to ensure continuity of care can lead to poor transition of care to lack of accountability.
