Additionally, the nurse facilitates transition of care through providing patient education on best self-management and self- care practices. What gaps can you identify in this process related to quality of care? (If you are not currently in practice, please use a previous role or clinical experience in your answers.) There are several gaps that emerge during care transition and these gaps could hurt continuity of high-quality care. One gap in patient transition inadequate patient education for older adults, particularly those without a family caregiver. This inadequate education is caused by memory decline or by ageism attitude mostly from the patients. According to Nemiroff (2022), ageism not only contributes to a lower quality of life for older patients, but can also result in missed or delayed diagnoses, more emergency room visits, more frequent hospitalizations and a shorter lifespan. Additionally, lack of a single point person to ensure continuity of care can lead to poor transition of care to lack of accountability. Besides, the lack of essential resources can cause safety lapses that can adversely affect care outcomes. References American Nurses Association (2021).
