Standard 5A: Coordination of Care Coordination of Care encompasses skills like planning, encouraging the patient to take care of themselves to reach goals, and arguing for the multidisciplinary team's provision of respectful, all-encompassing care. To provide patients with safer and more efficient treatment, care coordination in the primary care setting entails purposefully planning patient care activities and communicating information across all parties involved in a patient's care. Many persons may be a part of a patient's healthcare team if they have complex demands, such as those related to diabetes or other chronic conditions (Karam et al., 2021). Care coordination refers to how healthcare professionals collaborate to provide the appropriate care to patients at the appropriate location and time. To coordinate care, nurses use the following methods: sharing knowledge related to patient care with other care team members and working to ensure seamless care transitions (Karam et al., 2021). They can also collaborate with multiple providers on a healthcare team to develop a personalized, proactive care plan to manage a patient’s healthcare needs and support patients in managing their healthcare goals, including by connecting them with community resources. II. Standard 5B: Health Teaching and Health Promotion Health Teaching and Health Promotion are teaching strategies for health and wellness. One of the most critical roles nurses have in health promotion, and disease prevention is that of an educator. Nurses spend the most time with the patients and provide anticipatory guidance about immunizations, nutrition, dietary, medications, and safety. Patient education is an important component of nursing care and should be included during every patient encounter (Aqtam & Darawwad, 2018). For example, patient education may include teaching about side effects while administering medications or teaching patients how to self-manage their conditions at home. Competencies include health education needs assessment, program planning, implementation and evaluation, research, service coordination, and health educational needs, concerns, and resource communication no matter where the service is provided, such as in schools, communities, and clinics (Aqtam & Darawwad, 2018). b.
