Scenario: “65-year-old man presents to primary care center with a 5-day history of urinary dribbling, low back pain, peroneal pain, myalgias, and fevers with chills. He reports one episode of urinary incontinence this week. He denies any hematuria. Urinalysis with positive nitrites and trace bacteria. Patient has a history of a recent UTI. He also reports that his prostate has been enlarged and he had a recent transurethral surgery. Vital signs T 103.0 F, pulse 120, respirations 26.” The patient’s symptoms suggest that a urinary tract infection may be present and based on the history provided regarding his prostate the infection has spread to prostatitis, an infection of the prostate. This can happen due to many reasons in men of all ages and has been found to “affect 10 to 15 percent of the male population” (Prostatitis: Inflammation of the Prostate, 2022). When bacteria travels from the urethra into the prostate this type of infection will occur. The causes of prostatitis can be broken down between acute and chronic and bacterial versus non-bacterial infection known as chronic pelvic pain syndrome (Prostatitis: Inflammation of the Prostate, 2022). Bacteria can be transmitted from exposure to pathogens through a sexually transmitted pathway or via contact with uncleansed hands or surfaces. In this case this patient has a recent history of UTI and has recently had transurethral surgery putting him at higher risk for infections. The systemic reaction of fever, chills, tachycardia, and potentially sepsis are related to the purulent collection of infected fluid in the prostate related to an acute bacterial cause (Davis, 2023). If this condition is left untreated and antibitotic therapy is not initiated then an abcess can form creating further complications that may place the patient at risk for septic shock (Hazell, 2023). Inflammatory markers found to increase with sexually transmitted diseases or pelvic inflammatory diseases are typically CRP, ESR, white blood counts which confirm signs of infection and inflammation in the body (Yusuf & Trent, 2023). Specifically which chlamydia inflammatory marker CA-125 was noted to be significantly higher than those other infections (Park et al., 2017).
