NURS 6501 Week 4 Assignment; Case Study Analysis - Pulmonary Embolism (PE)

 According to the data provided, it seems that the patient may be experiencing pulmonary embolism (PE), a serious condition resulting from the obstruction of a lung artery by a blood clot. Some risk factors for PE comprise systemic lupus erythematosus (SLE), oral birth control use, prolonged immobility (such as airplane travel), and infection. The patient’s symptoms of dyspnea, left leg pain leg edema, and erythema are suggestive of PE, but other conditions may also cause similar signs and symptoms. Therefore, the patient needs urgent evaluation and treatment to confirm the diagnosis and prevent complications.  Some tests that may diagnose PE include a chest computed tomography (Ct) scan, D- dimer test, ventilation-perfusion (V/Q) scan, pulmonary angiography, and ultrasound of the leg veins. In some cases, thrombolytic therapy or surgical embolectomy may be needed to remove the clot more quickly. Duration and type of treatment may vary depending on the patient’s risk factors, severity of PE, and response to therapy.  SLE is a persistent autoimmune disorder that can affect several organs and systems inside the body. It can cause tenderness, damage, and problems in the skin, joints, lungs, blood, kidneys, brain, and heart. SLE can elevate the risk of infections also, osteoporosis, cardiovascular disease, and cancer. The symptoms and severity of SLE vary from person to person and may change over time. The treatment of SLE aims to control the disease activity, stop organ damage, and enhance the patient's standard of life. The treatment options comprise anti-inflammatory medications, immunosuppressive medications, biological agents, and hydroxychloroquine. The choice and dose of drugs depends on the patient’s symptoms, organ involvement, and side effects. The patients require constant monitoring also, and follow-up to assess the disease activity, organ function, and medication toxicity.  Oral birth control is a standard method of contraception that contains synthetic hormones, such as estrogen and progestin, that avert ovulation and pregnancy. Oral birth control can have also other advantages, like decreasing menstrual cramps, acne, and the risk of ovarian and endometrial cancer. Moreover, oral birth control can have some hard and side effects as well, such as elevating the risk of blood clots, especially in woman with SLE, antiphospholipid antibodies, or other clotting diseases. Oral birth control can as well interact with some medications used for SLE, such as warfarin and cyclophosphamide. So, women with SLE who utilize birth control needs to discuss the risks and benefits wit their doctor and consider alternative methods of contraception, such as progestin-only pills, intrauterine devices, or barrier methods.
