2 Osteoarthritis (OA) is a degenerative joint disorder primarily affecting cartilage, the smooth tissue that cushions the bones' ends within the joints. Over time, the cartilage breaks down, leading to pain, swelling, and stiffness. While OA can affect any joint, it most commonly joints such as the knees, hips, and spine. It is typically associated with aging, but other factors such as joint injuries and obesity can increase the risk of developing the condition. As OA progresses, it can significantly impact a person's quality of life, making daily activities more challenging. OA is characterized by the degeneration of cartilage and changes to the underlying bones. Its pathophysiology involves complex interactions between the joint's mechanical, biochemical, and cellular components. The hallmark of OA is the breakdown of articular cartilage, which cushions joints (Ouyang et al., 2023). Cartilage becomes thinner, roughened, and eventually eroded in advanced stages. This leads to the loss of smooth joint surfaces, causing pain and stiffness. Synovial cells produce inflammatory cytokines, prostaglandins, and other mediators that contribute to cartilage breakdown, pain, and the recruitment of inflammatory cells (Ouyang et al., 2023). In OA, the protective cartilage in the joints becomes worn down over time, leading to bones constantly rubbing together. This results in abnormal joint movements, which causes the characteristic grating sound known as crepitus. Crepitus is a sign of mechanical dysfunction within the joint and may indicate that the joint has reached a point of moderate to severe damage (Perumal et al., 2024). In addition to crepitus, joint space can be observed in patients with osteoarthritis. Joint space refers to the reduction in the distance between the bones in a joint, commonly seen in X-rays (Couch et al., 2024).
