2 1. Explain pathophysiologic mechanisms responsible for the patient symptoms and why patient is susceptible to more frequent infections. The symptoms presented by Michael are indicative of pulmonary fibrosis as a result of chronic exposure to silica dust. This is a case of occupational pulmonary fibrosis (silicosis) which is associated with progressive scarring of the lung tissue; therefore, undermining gas exchange while at the same time reducing lung compliance (Vanka et al., 2022). As a result of the underlying pathophysiology, this leads to restrictive lung disease; an aspect evidenced by a reduction in FVC and DLCO. Equally, his Velcro-like crackles are indicative of interstitial ling disease. This condition is associated with compromised mucociliary clearance as well as reduced immune defenses, therefore, leading to an increased susceptibility to frequent infections. Apart from this, Michael has a myriad of other factors that contribute to frequent infections. These include steroid therapy that has an immunosuppressive effect as well as chronic inflammation that promotes bacterial colonization and infection. Also, chronic hypoxia is associated with additional immunosuppressive effects (Vanka et al., 2022). 2. Explain the pulmonary pathophysiologic processes of how smoking can lead to the changes seen in the lungs. Michael has a long-term history of smoking unfiltered cigarettes. This is a major contributor to his lung disease as well as its worsening. Smoking is associated chronic inflammation and oxidative distress (Dunican et al., 2021). This has the overall effect of damaging the alveolar epithelial cells; an aspect that contributes to fibroblast proliferation. With time, this leads to the loss of ling elasticity which is further compounded by exposure to silica.
