Key Symptoms, Diagnostic Indicators, and Pathophysiological Mechanisms The patient presents with an acute onset of a swollen, erythematous, and painful left knee, inability to bear weight, and pain in the toe, a common initial site for gout flares. The symptoms began shortly after consuming alcohol over the weekend, with no trauma or systemic symptoms such as fever or rash. Laboratory findings reveal hyperuricemia (uric acid 8 mg/dL), elevated inflammatory markers (ESR 65 mm/hr; CRP 10.5 mg/dL), and the presence of monosodium urate (MSU) crystals in synovial fluid aspirate. This is confirmatory for diagnosing acute gouty arthritis (Rogers, 2023). Gout results from urate crystal deposition due to persistent hyperuricemia. Uric acid, a purine metabolism byproduct, crystallizes in joint spaces when serum levels exceed solubility thresholds. These crystals trigger the NLRP3 inflammasome, activating neutrophils and releasing cytokines (IL-1Ξ², TNF-Ξ±, IL-6), causing intense joint inflammation and pain (Dalbeth et al., 2021).
