2 Week Eight Case Study Analysis A 52-year-old male presented to the ER complaining of left great toe pain since yesterday. He reported a pain level of 10/10 and significant discomfort with a sheet touching his foot. He denied any recent injuries. The patient has a history of hypertension and high cholesterol. He is currently taking HCTZ and Simvastatin daily. His vital signs were as follows: His BP is 128/84, pulse is 70, resp 18, regular and non-labored, pulse ox 98%, and temp 100.1F. His physical exam was mainly unremarkable except for the first metatarsal phalangeal joint, which is erythematous and edematous. His uric acid level was 10mg/dl, and ESR was 32mm/hr. The x-ray of his left great toe showed normal joint spacing and no erosion. This paper aims to describe the pathophysiological processes leading to the patient’s symptoms, describe how the processes affect the patient, and explain any racial or ethnic variable that impacts physiological functioning. Pathophysiological Processes of the Patient Symptoms The patient in this scenario is likely experiencing gout. Gout is defined as an inflammatory response to high levels of uric acid in the blood or other body fluids (McCance & Huether, 2019). High uric acid in the blood is called hyperuricemia. High uric acid levels can also be present in other bodily fluids, such as synovial fluid. High uric acid levels can result in monosodium urate (MSU) needle-shaped crystal formation around or in joints (McCance & Huether, 2019). The MSU crystals trigger an inflammatory response within the joint. This leads to inflammation and erythema of the affected area; in this scenario, the affected area is the patient’s first metatarsal phalangeal joint. The pain this patient is experiencing is so severe that a sheet touching the area causes significant discomfort. The pain is likely due to the needle shape of the crystals pressing on the joint. The patient’s ESR level is elevated because of the 3 inflammatory response to the MSU crystallization in the joint. The patient’s uric acid level can be elevated due to the overproduction of uric acid or underexcretion. The underexcretion of uric acid can be due to impaired uric acid clearance in the kidneys leading to increased urate reabsorption (Fenando et al., 2024). The overproduction of uric acid can be due to increased purine synthesis, increased purine nucleotide breakdown, or HGPRT enzyme deficiency (McCance & Huether, 2019). The elevated uric acid levels could also be due to a side effect of the patient’s medication, HCTZ. HCTZ may lead to hyperuricemia, which may lead to gout if uric acid levels are elevated enough (Mayo Clinic, 2024). The patient’s vitals are all normal except for his temperature, which is 100.1F. His elevated temperature is likely due to the inflammatory response occurring in his body (Blomqvist & Engblom, 2018). How Do These Processes Interact? The processes of gout interact to affect the patient in this scenario. In gout, high uric acid levels lead to the needle-shaped crystallization of MSU, triggering an inflammatory response. This inflammatory response
